Frequently Asked Questions about in-home or post-acute care
Because we provide all the care necessary for post hospital recovery under one roof. This allows us to manage multi-disciplinary services internally and seamlessly. We have been a post-Acute care provider in the greater Houston area for the past 13 years. We are a preferred provider and trusted partner of all four hospital systems in the greater Houston area: Houston Methodist Hospitals, CHI St. Luke’s Health, Memorial Hermann Hospital System, and HCA Houston Healthcare.
The agency is licensed through Texas Health and Human Services, accredited through CHAP, and certified by Medicare. The agency currently holds a 4.5-star rating with Medicare (out of 5 stars).
Home care services or caregiver services are personal assistance services provided by unlicensed but experienced caregivers. These services are provided to clients to help them with accomplishing activities of daily living while maintaining their independence at home. Some examples of these services are companionship, bathing and grooming, dressing, toileting, feeding, and light housekeeping. Home health services are skilled and medical services provided by licensed clinicians to patients at their home. These services are provided to patients to continue their post-acute care and recovery after discharge from a hospital, a nursing home or a rehabilitation facility.
To receive home health services, patients must meet the home-bound status requirement. Home-bound status is defined by Medicare as, “those patients that require assistance when leaving the home and that when they do, it requires a considerable, taxing effort.” To receive hospice services, patients must have a diagnosis of a life-limiting illness or terminal disease with a life expectancy of six months or less as well as a desire for comfort care rather than curative care.
For home care or caregiver services, our agency accepts all private payment methods (cash, check or credit card) as well as payments from any long-term care (LTC) insurance contract.
For home health services, our agency accepts ALL Medicare and Medicare advantage plans. In addition to traditional Medicare plans, plans from the following insurances are also acceptable: United Healthcare, Aetna, Humana, Amerigroup, Molina, and Tricare (Humana Military). We accept private payments for home health services for patients who do not have any insurance. We also accept charity or unfunded patients on case-by-case basis.
For hospice services, we currently accept private payments (cash, check, credit cards) only. Very soon, we will be accepting Medicare and Medicaid plans as well.
Now. It is never too soon to establish a relationship with a caregiving agency. Get to know what resources are available to you before it becomes emergent. Start by identifying areas that have become tasking for our seniors to complete safely. In these more difficult tasks lie the areas where we can reduce the risks of falls or missed showers. It is always a good time for companionship.
Home care or caregiving services can begin as soon as the same day of inquiry. Depending on the scheduling needs of our clients, we are able to begin ASAP.
The following documents are required for a good referral and for services to start as fast as possible: the patient’s demographic or information sheet, a physician’s order for home health or hospice, and the most recent progress note signed by a physician.
Anyone can refer a patient to our services for home care, home health or hospice. This includes referrals by the patient, a doctor, a clinician or case manager, a social worker, a family member, a neighbour, or even a friend.

The following steps are the fastest and easiest way to sign up for our services: a. Contact our office to determine the specific need and choose the right services for the patient; b. Get the patient’s primary care physician (PCP) to send our office the necessary documents (patient’s demographics, home health or hospice order, and latest progress note); c. Allow 24 hours for our clinical staff to contact the patient or the patient’s representative to schedule the first assessment visit at the patient’s home.
Our agencies perform a 5-point verification process upon being hired. We verify compliance on an annual basis thereafter. Through our thorough hiring process, we are able to maintain a trusting environment.
